This document includes details about how the Adventist HealthCare Retirement Plan (AHRP) works and rules that apply to your AHRP account.
- Payment Rights Notice and Special Tax Notice (PDF)
This notice gives you tax information you need to know when you take money out of your AHRP account.
- Signature Authorization/Enrollment Form (PDF)
You must fill out and sign this form to enroll in the AHRP and to authorize payroll deductions and transactions.
- Rollover Form (PDF)
This form allows you to rollover a retirement account from a previous employer into the AHRP.
You cannot complete these forms on-line but you may print, fill-out and return the forms to your Human Resource Office or send them directly to the AHRP Retirement Center at the following address:
